Are Beam Hours Important When Purchasing A Used LINAC?

Buying used and refurbished linear accelerators can save medical facilities a great deal of money.  This is fairly critical when it comes to the limited budgets that growing healthcare centers face.  The question most often asked is about the HV (high voltage) hours, beam hours, and how important their role is in determining the wear and tear of a LINAC.

The most universal analogy comes down to the mileage of a used vehicle, the more miles usually means more wear and tear.  The higher the mileage, the less you should expect the resale value to be.  Beam hours are very similar.  Beam hours are equivalent to the time that the beam has been in use.  Thus, higher HV hours often means that these machines will be priced lower than LINACS with lower beam hours.  This of course assumes all other things are equal.

In reality a dozen factors go into determining the price facilities will pay when purchasing a used linear accelerator.  These include factors such as manufacturer, age of LINAC, updates to software/technology, and upgrades to the machine.  The chart below offers a reasonable explanation to what facilities should expect in regard to beam hours.

On the top you will see a number that reflects the years a used machine is expected to be in place at the new facility.  When a facility is getting started, doesn’t expect to see a lot of patients, and expects to have a higher revenue to purchase equipment in the next couple of years a used LINAC with higher beam hours is acceptable to be purchased.   However, if this same facility hopes to hold onto the machine for an extended period of time or expects to see a high number of patients, they should re-evaluate the number of acceptable beam hours that can be on a used LINAC. 

Situation: A health care center looking into adding a cancer treatment center to an already booming facility.  They are expecting that as they make available this new treatment option that their revenue will increase.  They also expect that as patients become aware of the option for treatment the number of patients, they can expect to see should be higher than they are to begin with.  They want to purchase a LINAC without having to replace it for about 10 years.  What amount of beam hours should they be looking for on a used LINAC? 

This facility, because they are assuming, they will see a high number of patients for treatment as the word spreads that they are offering LINAC treatment should assume that in time they will be seeing more than 10 patients a day.  Even if they start out slow at 5 patients a day now, it is assumed that they will be treating upwards of 15 or more eventually which will average out.  Because they want the equipment to last at least 10 years this facility should only be looking at LINAC systems with between 1000-1500 used beam hours anything with more than that will not serve the purpose for the volume of patients they are expecting or the years that they wish to use this piece of medical equipment.

Another thing a facility needs to consider is how often IMRT and VMAT treatment will be used.  These two types of treatment options consumer more beam hours.   This should be considered when looking into a used LINAC.

Software Development For iPhones and Android Using Cordova

One of the greatest challenges of a software development company working with iPhone and Android apps is time to develop on both platforms. While native programming these devices has its positives, rapid deployment of software is crucial in maintaining a profitable business model.  Using Cordova (PhoneGap) to achieve this, we have realized 200-300% reductions in time to market. Several of our apps run on Cordova and are available in the Apple Store, Google Play and Amazon.

Cordova allows you to keep one set of source code for both platforms, utilizing HTML and JavaScript.  This means you can simply develop your software one time for both devices and deploy.  Cordova comes with a breath of plugins from camera management, screen rotation, keyboard pop up, uploading file and more.  You install Cordova, add required plug-ins and develop.

If you really want to speed up design time, you can actually develop your phone or tablet app on a website, using Chrome, FireFox or Edge.  Utilizing the CSS and HTML power of real time editing, you will see many apps can be produced in 1-2 days.  This allows for greater profit margins, less stress, and an ease on deadlines.

We have developed a technique to do just that.  By using the code below, you will disable Cordova (PhoneGap) in the browser view.  Then you upload the www project files to a website you have control of and can edit it right there.

This code works by checking the URL of the device.  When running in app mode (on a device) the system does not use http protocol.  When in the browser we do.  This code simply checks to see if we are on a browser or not, then includes the cordova.js file.

This will allow you to program your app without the errors in the console on a website.  To continue to speed up development, we utilize jQuery.  If you are not familiar with jQuery, it’s a library of incredible useful shortcuts and tools for JavaScript.

I highly suggest you rip out much of the default code that comes as a new project in Cordova.  The only thing you really need to include is the cordova.js file.  Using simple document ready function for the JavaScript replaces the cumbersome device ready code provided in the project.  Using jQuery $(document).ready() will fire when the page is loaded.

Using a combination of the process above, one should be able to rapidly deploy apps for home or business use, in a stable and reliable environment.  One of our big concerns when we first started this process was how Apple would react to an app not “natively” programmed in XCode.  The team at Apple will not reject your code because you used Cordova.  We have multiple apps in the Apple Store programmed using Cordova.  Give it a try, you will not be disappointed.

Electrical Distribution Equipment Failure – Keep or Discard

When dealing with electrical equipment and components it is inevitable that they will fail, and you will have to deal with the consequences.  The minute electrical equipment is put into place, from transformers to circuit breakers and panel switches and tap boxes, they start to decline.  Deterioration of electrical products happens for a number of reasons including:

  • Moisture Absorption
  • Differing Temperature Cycles
  • Dust and Particles Settling
  • Condensation Buildup
  • Fragile Operating Springs
  • Insulation Material Breakdown
  • Rusted Out Switchgear Enclosures
  • Dried Out Capacitors

When electrical components within distribution systems age vulnerabilities start to appear. With automation being what it is today, electrical technology can often run unassisted, without issue for years. Because of this maintenance on these systems tend to be ignored which can lead to unpredicted failures which lead to catastrophic failures; the most common being fire.  Thus, in order to keep electrical components operating at their peak and delivering consistent power renovations and maintenance are critical.

The two biggest nemeses of electrical equipment and the components within are moisture, condensation, dust, and loose particles. When equipment is installed outdoors or in spaces that offer no type of temperature or humidity control deterioration occurs more rapidly then in spaces where there are controls, just at a slower, more gradual pace. 

Based on the quality, maintenance schedule, and environment of electrical equipment and components that are installed the deterioration rate varies.  When planning for maintenance, upkeep, and refurbishment of electric components and equipment it is important to establish the age and overall condition of each piece. 

Electrical Equipment and Component Insulation

Components within each piece of equipment must be inspected individually.  Insulation is the component of electrical equipment that degrades the quickest.  Paper and solid synthetic insulation is used throughout equipment around motors, capacitors, cables, transformers, circuit breaker trip coils, operating coils of contractors, and reactors.  The breakdown of insulation depends a great deal on the temperatures that it is exposed to.  To increase longevity of equipment and decrease the breakdown of insulation it is important for equipment not to be operated above optimal temperature ratings.

Capacitors in Electrical Equipment

There are various types of capacitors used in both low and medium voltage distribution systems including:

  • Surge Capacitors
  • Power Factor Correcting Capacitors
  • Commutating Capacitors
  • Capacitors in Active and Passive Filters
  • Pole-Mounted Capacitors

The most common capacitor to use in ratings up to several hundred kVAR is power factor correction.  They are sealed in airtight, insulated units for protection.  The insulating material that is used is a metalized polypropylene film that is compressed in a thermal setting resin.  In the event of internal failure, pressure sensitive interrupters are used to disconnect the capacitor. Capacitors are always fused externally because their failure is most often due to circuit shorts.

There is a nonstop loss of power internally because of lag thus causing the insulating resin to dry.  This often goes unnoticed.  Most capacitors have an average life expectancy of about seventeen years when in continuous operation. This of course decreases depending on the environment in which it operates.  Many systems have a decrease in this average life expectancy due to being exposed to over use and variation in frequency.  To check measurements for capacitors, use clamp-on ammeter to test and detect disparity over time.

There are many reasons that health care facilities look into purchasing refurbished linear accelerators verse new.  Most assume that refurbished medical equipment is purchased solely because it is more affordable than new however that is not the only reason to consider. 

Look into refurbished LINAC systems when:

  • Treating fewer than ten patients a day

When medical facilities are not exclusively treating patients seeking radiation using a linear accelerator there may not be the funds to support the expense.  Health care centers that treat less than ten patients a day using a LINAC benefit from purchasing a refurbished system.  A previously owned, refurbished linear accelerator is less expensive and thus the monthly expense to lease/finance is lower.  What this means for your facility is that the payment is able to be covered by the few patients being treated daily.

  • Nearsighted patients that don’t need reading glasses before the surgery might need them after.

LASIK benefits may decrease overtime.  In fact, more than 10% of patients may require a second surgery which is known as a retreatment.  This will restore the patients desired vision.  This process is more common in patients who had higher nearsighted or farsightedness or a higher degree of astigmatism.

Steps Involved In Cleaning Your Water Softeners Brine Tank

Part of owning a water softener is maintaining it.  One aspect of water softener maintenance involves cleaning the tank which holds the brine.  Cleaning a water softener tank should be done every five to ten years.  It is time to clean the tank when your water turns hard and the basic tank maintenance doesn’t fix the issue. Older water softening units can benefit from annual cleanings especially electric water softeners. 

The first step in cleaning the water softener tank involves emptying the tank.  This is done by locating the bypass valve and shutting off the water intake.  Water can be siphoned out of the tank.  After this process is completed you will want to remove the leftover salt and safely throw it away.  Water softener salt must be disposed of away from plants.  Water softener salt will kill plants and other landscaping plants and shrubs. Another way to get rid of large blocks of salt or bridges is to dissolve it in hot water. 

Once the water and salt have been removed from the tank homeowners should remove the brine grid that sits at the base of the tank.  Some water softening units have a mesh platform instead.  Set this grid to the side before cleaning the tank.

Once the water softener tank is empty and the brine grid or mesh has been removed it is time to clean the tank with a good amount of soap and water.  This mixture should be scrubbed all around the interior of the tank with a long-handled scrub brush.  After the tanks has been thoroughly cleansed the soapy water can be dumped out and rinsed with clean water.

After this has been done the homeowner should clean the tank with a combination of bleach and water. Stir the bleach/water combination in the tank and let it sit for fifteen minutes to completely disinfect the tank.  Organisms have a difficult time to grow in the concentrated brine however the bleach and water mixture makes sure.

Once this process has been completed and the bleach/water combination has been rinsed out it is time to replenish the tank with water and salt.  First replace the grid into the bottom of the tank.  Add water softener salt and water back into the tank when it is back in place.  Give the tank a couple of hours before you regenerate the water softener as this time is needed in order to dissolve the salt within the tank.

Routine Spring Maintenance for the Interior and Exterior of Your Home

It is crucial for homeowners to perform thorough maintenance on their homes at the beginning of the spring season.  Once the winter weather thaws and the sun starts peaking through it is easier to notice issues where your home could benefit from some attention.

Roofing

Roofing contractors are an important element in the inspection of your homes roof.  We all like to think we could spot a problem with our roof however it is not always so simple especially for the novice eye.  When shingles are missing or have become cracked it is not always seen from the ground.  A roofer can spot these and replace them with a rooftop inspection.  If shingles have shifted it is important for a roofer to install new fasteners.  It is also crucial that the build up of pine needles, leaves, and debris in the valleys and peaks of your homes roof is removed and not allowed to settle in the home’s gutters.

Chimney

Brick chimneys can also benefit from springtime maintenance.  It is important for homeowners to check out the chimney material whether it is brick or stone.  How are the joints, is there loose mortar?  Have weeds started growing, is there evidence of water, or a white calcium deposit?  These are all signs that your chimney needs maintenance. 

Foundation

Your homes foundation is the most important element as it keeps your home standing.  An inspection of your homes concrete foundation should look for cracks of all sizes.  If there is a crack in the foundation you will need to higher a foundation specialist to bond the crack chemically.  A two-part epoxy injection system is used to ensure the crack will not further expand. 

Windows

Windows with leaks in the seal around them will let warm air in during the summer and cool air to escape in the winter.  It is important to check the caulk and weather stripping to make sure it has remained in place throughout the winter.  Condensation on the inside of the double-glazed windows during the month often means that the weather seal has been compromised.  This means that the window will need to be replaced. 

Leaky Faucets

Spring is the perfect time to inspect faucets inside and outside of your home.  Check for leaks, clogs, and sweaty pipes.  Look under your sinks for proper connections to ensure there are know leaks coming from underneath.  Examine dishwashers, washing machines, and other appliances with sources of water for staining.  Repair or replace hoses and pipes for cracks, bulges, and/or dampness.

Air Conditioners

Just as homeowners are expected to inspect their homes heating systems in the fall, they should inspect their cooling systems in the spring.  Air conditioners need to be in good working order to ensure they are ready for the heat of the summer.  Change out the systems filter, check hoses and connections and ensure the drain is able to freely drain. 

Homeowners should also inspect and perform maintenance on their attics, windows, and basements.  Your home is your most valuable asset and should be cared for as such.  A little general maintenance a couple of times a year can increase the longevity of the systems that are in place.
